Background Check Authorization

Adverse Action Notice

If we take adverse action based in whole or in part on information contained in your consumer report, you will receive notice of such action along with information about the consumer reporting agency that provided the report.

Authorization to Obtain Consumer Report

I hereby authorize Trustrly and its designated agents and representatives to conduct a comprehensive review of my background through a consumer report and/or investigative consumer report for employment purposes.

Information I Authorize to Be Released

I understand that the scope of the consumer report may include, but is not limited to:

• Social Security number verification
• Criminal record checks (federal, state, county)
• Sex offender registry searches
• Employment history verification
• Education verification
• Professional license and certification verification
• Motor vehicle records (if applicable)
• Credit history (where permitted by law)
• Civil court records
• Reference checks

Release of Liability

I release and agree to hold harmless Trustrly, its agents, employees, and any person or entity that provides information pursuant to this authorization from any and all liability arising from the use, disclosure, or release of such information.

Understanding of Rights

I understand that:

• I have the right to request a copy of any consumer report obtained
• I may dispute any inaccurate information in the report
• I will be notified if adverse action is taken based on the report
• This authorization remains in effect during my employment

Certification of Accuracy

I certify that all information provided by me in connection with this authorization is true, accurate, and complete to the best of my knowledge.
